Instructions to Request an Official Transcript by Fax

Follow these instructions to request an OFFICIAL transcript. Unofficial transcripts (for attendees since September 1984) can be viewed and printed from this website by selecting MyBanner option on the Registrar's Office homepage.

Send your faxed transcript request to (616) 331-2000. You must include the following information on your fax or complete the Transcript Request Form.

• Name(s) you used while attending Grand Valley State University
• Approximate dates of attendance
• Banner G number or the last 4 digits of your social security number
• Birth date
• Number of copies
• Your current address
• Your daytime phone number
• Complete address of transcript(s) destination (transcripts cannot be emailed)
• Your signature (required to process request)
